CPA Examination

Education Evaluation Worksheet:  (8½" x 11" paper size, 1 page) Use this worksheet for self-evaluation of your foreign education evaluation and/or U.S. college transcripts before you apply for the CPA exam. 

License

License Application - (8½" x 11" paper size, 3 pages)  Use this form to apply for a Washington CPA license (including by transfer of credits).  NOTE:  As of July 1, 2001 the Board will no longer issue certificates.  All applicants must meet the education, exam, ethics, and experience requirements in order to apply for a license.

Certificateholder License Application - (8½" x 11" paper size, 2 pages) If you currently hold a valid Washington CPA-Inactive certificate, use this form to apply for an initial license to practice public accounting.

Application for License of Certified Public Accountant by Interstate Reciprocity:  (8½" x 11" paper size, 6 pages)  Use this form to apply for a Washington CPA license based on a license issued by another State Board of Accountancy.

Application for License of Certified Public Accountant by International Reciprocity:  (8½" x 11" paper size, 3 pages)  Use this form to apply for a Washington CPA license on an Board recognized accounting credential from another country.

Application to Return to Previously Held Status as a Licensee; (8½" x 11" paper size, 2 pages)  A CPA-Inactive certificateholder will use this form to return to a previous status as a licensee.  Do not use this form if you have never held a license in Washington State or if your certificate/license has lapsed.

Experience Affidavit  Effective December 16, 2003 -  (8½" by 11" paper size, 6 pages)  Use this affidavit to show completion of the work experience requirement.  Additional helps:

Sharing Exam/Licensing Information Between State Boards

Request to Transfer Exam and Licensure Information to Washington State  (8½" x 11" paper size, 2 pages)  Use this form to request another State Board to provide Washington State with:

  • Exam scores, or
  • Licensing information for holders of a CPA certificate/license/permit in another state (Licensing information includes issue date, exam scores, ethics exam, absence of disciplinary proceedings, and good standing with the home state)  

Do NOT use this form to request the Washington State Board of Accountancy to provide exam or licensure information to another State Board or professional association.

Request to Provide Exam and/or Licensure Information From Washington State to Another State Board (8½" x 11" paper size, 1 page)  (Effective August 1, 2007, no fee is charged to process this request.) Use this form to request the Washington State Board of Accountancy to provide your Washington State exam and/or licensure information to another state board.  

Authorization for Exchange of Professional Accounting Credential Information for International Reciprocity   (8½" x 11" paper size, 2 pages)   Use this form to verify your accounting credential (for example, Provincial Institute of Chartered Accountants).

Renewal

Application for Renewal of Washington State CPA License, CPA-Inactive Certificate, or Resident Nonlicensee Firm Owner- The Board mails renewal forms in January of the year of renewal directly to the last address of record (2007 Renewal Forms mailed January 12).  If your CPA license, CPA-Inactive certificate or nonlicense firm owner registration expires on June 30, 2007, you should receive your renewal form in mid January 2007.  If you do not receive a form by mid February, it is your responsibility to contact the Board office to obtain a replacement.  If you failed to renew your CPA-Inactive certificate or license that expired June 30, 2006, you must apply for reinstatement.  Reinstatement forms are available below.  Or, please contact Customer Service at customerservice@cpaboard.wa.gov.

Retired CPA-Inactive Certificate Renewal Application (8½" x 11" paper size, 2 pages)  If you retired your CPA-Inactive certificate, use this form to return to your previously held status as a CPA-Inactive certificateholder.

Retired License Renewal Application (8½" x 11" paper size, 2 pages)  If you retired your CPA license, use this form to return to your previously held status as a licensee.

Retirement

Application for Retired License or Certificate Status (8½" x 11" paper size, 1 page)  Use this form to retire your CPA license or CPA-Inactive certificate.

Reinstatement

License Reinstatement; (8½" by 14" paper size, 3 pages); Use this form to apply for reinstatement of a lapsed, revoked or suspended license. (Note, to print 8½" by 14" forms properly, you must set your printer settings for legal sized paper.  Click on Acrobat's printer icon (NOT your browser's printer icon), choose Properties in the Print dialogue box, change Paper Size to legal, and Paper Source, if necessary.)

Certificate Reinstatement: (8½" by 11" paper size, 3 pages); Use this form to apply for reinstatement of a lapsed CPA-Inactive certificate (Note: Reinstatement of a CPA-Inactive certificate is only available for persons whose CPA-Inactive certificate lapsed on or after July 1, 2001.)

Certificate Reinstatement: (8½" by 11" paper size, 3 pages); Use this form to apply for reinstatement of a suspended or revoked CPA-Inactive certificate (Note: Reinstatement of a CPA-Inactive certificate is only available for persons whose CPA-Inactive certificate was suspended or revoked on or after July 1, 2001.)

Continuing Professional Education (CPE)

Worksheet (8½" x 11" paper size, 1 page)  Use these worksheets to evaluate whether you have met the CPE requirements during a reporting period.

  • January 1, 2005 - December 31, 2007 Worksheet (for renewal in 2008)
  • January 1, 2006 - December 31, 2008 Worksheet (for renewal in 2009)

Waiver Request   (8½" x 11" paper size, 1 page)  If you believe you are unable to meet the Board's CPE requirements and you qualify for an exception, use this form to request a waiver to the CPE requirements from the Board.

CPA Firm

CPA Firm License Application - With office(s) in Washington State (8½" x 11" paper size, 3 pages)  Use this form to register your CPA firm (sole proprietorship, partnership, professional service corporation, or limited liability company) with the Board.

CPA Firm License Application - With no office(s) in Washington State (8½" x 11" paper size, 3 pages)  Use this form to register your CPA firm (sole proprietorship, partnership, professional service corporation, or limited liability company) with the Board.

CPA Firm License Amendment (8½" x 11" paper size, 3 pages)  Use this form to notify the Board of:  the admission or departure of an owner; any change in the name of the firm; or a change in the managing licensee of the main office.

Ownership Listing Form (8½" x 11" paper size, 1 page)  Use this form to report non-CPA firms owners of a CPA firm.

Registration to be a Nonlicensee CPA Firm Owner (8½" x 11" paper size, 3 pages)  Use this form to register to be a Nonlicensee owner of a CPA firm.

Firm License Renewal Application - Not available - The Board mails renewal forms in January of the year of renewal directly to the last address of record.  If your firm  license expires on June 30, 2007, you should  receive your renewal form in late January  2007.  If you do not receive a form, it is your responsibility to contact the Board office to obtain a replacement.  Please call (360) 586-0952.

CPA Firm Change of Address Form  (8½" x 11" paper size, 1 page) Use this form to change the address of a CPA firm or to provide notification of the opening, relocation or closure of a branch office.

Relinquishment of Firm License (8½" x 11" paper size, 1 page) Use this form to relinquish a CPA firm license.

Compensation

Disclosure Forms - (8½" x 11" paper size, 3 pages) Use these forms to provide clients with the required disclosure contained in WAC 4-25-626.

Reportable Event

Notification of a Reportable Event - (8½" x 11" paper size, 3 pages) Use this form to notify the Board of a reportable event (that is, the issuance of an order or charges, etc.) as required by  WAC 4-25-670.

Complaints

Complaint Form - (8½" x 11" paper size, 3 pages) Use this form to file a complaint against a CPA or CPA firm.  This is a fill-in form.  You may complete the form, save, print, and then submit it to the Board.

General

Individual Change of Address   Use this fill-in form to provide your address change notification in writing to the Board.  Please note, CPA firms changing their address must use the CPA Firm Change of Address form above.

Name Change/Wall Document Replacement Affidavit  (8½" x 11" paper size, 1 page)  Use this form to change your name and/or replace your CPA wall document.
